Aviation safety measurement systems focus on collecting and reporting trailing edge data, capturing accident, incident, occurrence, threat and error, non-conformance and non-compliance data through open reporting, FDR and audit programs. This data reveals “what has happened” but is a limited gauge as to assessing the probability of “what will happen”.
